Combine water, granulated sugar, and fresh blueberries in a medium saucepan over medium to medium-high heat. Bring to a boil, then cook for 5 minutes, or until the sugar is fully dissolved and the berries burst. Gently mash the blueberries to release their juices.

Remove the pan from heat. Add basil leaves, stir, cover with a lid, and allow to steep for 20 minutes to infuse flavors.

Strain the mixture through a fine mesh sieve into a bowl or jug, pressing the solids to extract as much flavor and color as possible. Discard any remaining solids.

Pour the blueberry-basil syrup, freshly squeezed lemon juice, and chilled sparkling water into a large pitcher. Stir thoroughly until well combined.

Serve over ice and garnish with lemon slices, whole blueberries, or basil leaves as desired.
